These are random breakout boards for pedals that I have made and are available from OSHPark. I recieve no money from orders - the "OSH" stands for Open Source Hardware.
Easily rearrange substituted to-92 transistors to different pinouts. Each row moves a different pin to the middle. Order on after dark so the traces are more visible. Laid out like this:

Place in the row that matches the pinout, and then add headers or buss wire into the row that matches the desired pinout in your board.
These are also available under different names, and can be found on AliExpress for next to nothing. They are slimmer than other shrouded 1/4" jacks although a little bit more flimsy. Pictured next to a typical shrouded jack.
